Arrows scroll up the screen in four lanes and you press the matching key as each one reaches the top row. Hit them and the health bar leans your way. Stack up misses and it tips to your opponent and the song ends early.

Heartbreak Havoc plays Wife Forever, Sky and Manifest back to back on Hard, over a sky-high stage that keeps drifting past while you try to keep up. All three are also available on their own in Free Play, which is where you will want to practice, because there is no Easy option to hide behind.
